I can't answer the first two polls! Though I'm probably being too picky with #1. I use Notetab, which is a text editor similar to Notepad, only a whole lot more versatile - multiple tabs, adds html, is generally darn nifty. (And I used to use Word, but due to security restrictions at work, I can't really work with Word files on my jump drive anymore. Another reason to favor text editors.) Annnnd if I use a beta, it's usually my friend lemniskate, so I post it on LJ filtered to her.
It works for us because we're usually chatting in AIM at the same time, so she'll just tell me what she sees. =) If I were working with someone else, I'd probably email it. I like notes, rather than having my text changed, so I can choose what to change based on others' input.
i was going to say the same about my stuff. if i post on lj, it's generally on my writing journal, which means it's filtered to *other* writing journals. granted, no one really betas me anymore, not unless it's little dinky spelling errors my brain just couldn't handle. but my best friend from high school still looks over stuff once i post it, tells me what's wrong, and i go back and fix it both in the hard copy on my hard drive and the online copy on LJ.

I've thought of the LJ filter thing, but I like betas to poke around and do things to the text, as long as they mark the changes.
