Yeah, I mean technically, my thought is that he was Adam when Rhys opened the door, and physically altered his appearance after Rhys touched him.
So it's Ianto in terms of, well, photos of Ianto and things Rhiannon has. Thinner because Ianto was a bit thinner when Adam actually saw him.
But it could be Adam, physically. Whatever you like, really. :D (Bryan Dick is GREAT.)
